The EPA certification form you sign at refrigerant purchase has specific fields that matter. Here's the line-by-line explanation.

When you buy refrigerant from a legitimate supplier, you'll certify your eligibility by signing a form that your supplier keeps on file for three years per 40 CFR §82.154. The form is not complicated, but several fields trip up contractors regularly.
Field-by-field
- Purchaser name: Full legal name matching your EPA 608 / 609 certificate.
- Company: Business that employs you or is purchasing.
- EPA certification number: The number on your 608/609 wallet card or certificate.
- Certification type: I (small appliance), II (high-pressure), III (low-pressure), Universal (all three), or 609 (mobile AC).
- Certification expiration: Many 608 certs don't expire; enter 'does not expire' if applicable. Some state renewal requirements exist.
- Wholesaler resale box: Check only if you are a registered refrigerant reseller buying for resale to other certified techs.
- Signature and date: Your signature - not a rubber stamp.
Common mistakes
- Checking the wholesaler box when you're not a reseller - this is falsification and risks the supplier's license
- Using an expired cert number - EPA cross-references; the supplier can be fined
- Having a purchasing manager sign when the cert holder is someone else - the signature must be the cert holder's
