EPA Form Walkthrough - Filling It Out When You Buy Refrigerant

When you buy refrigerant from a legitimate supplier, you'll certify your eligibility by signing a form that your supplier keeps on file for three years per 40 CFR §82.154. The form is not complicated, but several fields trip up contractors regularly.

Field-by-field

  • Purchaser name: Full legal name matching your EPA 608 / 609 certificate.
  • Company: Business that employs you or is purchasing.
  • EPA certification number: The number on your 608/609 wallet card or certificate.
  • Certification type: I (small appliance), II (high-pressure), III (low-pressure), Universal (all three), or 609 (mobile AC).
  • Certification expiration: Many 608 certs don't expire; enter 'does not expire' if applicable. Some state renewal requirements exist.
  • Wholesaler resale box: Check only if you are a registered refrigerant reseller buying for resale to other certified techs.
  • Signature and date: Your signature - not a rubber stamp.

Common mistakes

  • Checking the wholesaler box when you're not a reseller - this is falsification and risks the supplier's license
  • Using an expired cert number - EPA cross-references; the supplier can be fined
  • Having a purchasing manager sign when the cert holder is someone else - the signature must be the cert holder's
